Advantages

PCVの利点

One of the advantages of pressure targeted ventilation is that the peak airway pressure is controlled. If the clinician sets the inspiratory pressure, it should remain constant at that level throughout inspiration. This is one method of controlling the incidence of barotrauma, such as pneumothorax.

　圧規定換気の利点の一つとして、最高気道内圧を調節できることが挙げられます。吸気圧を設定すれば、吸気の間中そのレベルで一定に保たれるはずです。これは気胸のような圧損傷を予防する方法のひとつです。

Another advantage of pressure targeted ventilation is that the inspiratory flowrate is variable. If the patient has a variable inspiratory effort, it may reduce the patient’s work-of-breathing to use pressure targeted ventilation which allows the patient to vary the inspiratory flowrate. Many clinicians feel that this is a very important advantage of pressure targeted ventilation.

　圧規定換気のもうひとつの利点は吸気流量が変化することです。患者の吸気努力が変化しているときには、患者が吸気流量を変えることができる圧規定換気を使用することにより、患者の呼吸仕事量を減少させられるかもしれません。多くの臨床家はこの点が圧規定換気の大変重要な利点であると感じています。

Disadvantages

PCVの欠点

As mentioned above, one of the advantages of pressure targeted ventilation is that the flowrate is variable. This also leads to a disadvantage because the tidal volume is not controlled. It may allow the patient to receive a tidal volume that is inappropriately high or low. In addition, because the tidal volume is not well controlled, it is more difficult to control the alveolar ventilation and subsequently the PaCO₂ and pH.

　上に述べたように、圧規定換気の利点のひとつに流量が変化することが挙げられますが、これはまた一回換気量を調節できないという欠点にもつながります。一回換気量が不適切に高くなったり低くなったりする可能性があります。さらに、一回換気量を調節できないために、肺胞換気とさらにはPaCO2とpHを調節することがより困難となります。

Another possible disadvantage of pressure targeted ventilation is related to the complexity of understanding pressure targeted ventilation. There are 10 – 15 options for changes in ventilator settings to increase or decrease the minute ventilation in pressure targeted ventilation, requiring a better understanding of physiology and ventilator mechanics as compared to volume targeted ventilation.

　圧規定換気の欠点がもう一つあるとすると、理解するのが難しいことです 。圧規定換気において、分時換気量を増加もしくは減少させるための人工呼吸器設定の変更方法には10〜15通りもあります。そのため量規定換気と比較すると、生理学と人工呼吸器の仕組みをよりよく理解する必要があります。
