[アシュワース教授] パート3その2 オートPEEPを発見するためのフロー−時間波形

前回の続きです

When you look at the Flow versus Time Waveform in the following image, you will notice that the expiratory flow does not return to baseline before the next breath begins. Therefore, the patient has AutoPEEP.

 以下の図中のフロー−時間波形を見た時、呼気のフローが次の吸気が始まる前に基線に戻っていないのに気づくでしょう。つまりこの患者にはオートPEEPが起こっています。


Sometime it is difficult to determine whether or not AutoPEEP is present and it is helpful to change the scale of the Flow versus Time Waveform. In the following image, the scale was changed to allow the clinician to more closely examine the Flow versus Time Waveform, and determine that AutoPEEP is present, as expiratory flow does not return to baseline before the next breath begins.

 時々、オートPEEPが存在するか否か判断が困難な時があります。その時はフロー−時間波形のスケールを変えるとよいでしょう。次の図の中では、呼気フローが次の吸気が始まる前に基線に戻っていない時、臨床家がフロー−時間波形をより詳細に観察でき、オートPEEPが存在するかどうか調べられるようにスケールが変えています。


It is not possible to measure the AutoPEEP with the Flow versus Time Waveform. You can only determine whether or not AutoPEEP is present. If the clinician wants to measure the AutoPEEP, the clinician can provide an expiratory pause (expiratory hold) and measure the airway pressure at the beginning of the next breath. During this procedure, the patient must not make any inspiratory efforts.

 フロー−時間波形でオートPEEPの値を測定することはできません。オートPEEPが存在するかどうかを判断するだけです。もし、臨床家がオートPEEPの値を測定したいならば、呼気ポーズ(呼気ホールド)を使用し、次の吸気の始めの気道内圧を測定することができます。この過程において、患者は吸気努力をしてはいけません。

As mentioned previously, AutoPEEP may be reduced by decreasing airway resistance or increasing expiratory time. In most modes it is possible to increase the expiratory time by decreasing inspiratory time or decreasing the respiratory rate.

 上で述べたように、気道抵抗を低下させること又は、呼気時間を延長させることでオートPEEPを減少させることができます。ほとんどのモードで吸気時間を減少させる又は呼吸回数を減少させることで呼気時間を延長させることが可能です。

The Flow versus Time Waveform can be used to detect the presence of AutoPEEP. AutoPEEP, frequently called air-trapping, results when the patient is not given enough time to exhale back to baseline. This results in an increased Functional Residual Capacity (FRC), increased mean airway pressure and may make it difficult for the patient to trigger additional breaths from the ventilator.

 フロー時間波形はオートPEEPが存在するかを見る時に使用することができます。オートPEEPはしばしばエアートラッピングと呼ばれますが、基線に戻るまでの十分な呼気時間が患者に与えられない時に起こります。これはFRC(機能的残気量)の増加を引き起こし、平均気道内圧を上昇させます。そして患者が人工呼吸器からの呼吸をトリガーすることを困難にさせてしまう可能性があります。


When patients have an increased airway resistance (possibly due to bronchospasm, secretions, small endotracheal tube, mucosal edema) or an increased compliance (possibly due to emphysema), they are more likely to have AutoPEEP, if the expiratory time is not long enough. Methods of reducing AutoPEEP include reducing airway resistance and lengthening the expiratory time. In most ventilator modes, the expiratory time can be increased by decreasing the inspiratory time or decreasing the respiratory rate.

 患者の気道抵抗が増加した時(気管支攣縮、気管支内分泌物、小さい気管内チューブ、粘膜浮腫等のため)、またはコンプライアンスが増加した時(肺気腫等のために)、呼気時間が十分長くない場合はオートPEEPを起こしやすくなります。オートPEEPを減少させる方法としては気道抵抗を減少させる事と呼気時間を長くする事が挙げられます。ほとんどの人工呼吸器のモードでは吸気時間を減少させるか、または呼吸回数を減少させることで呼気時間を長くすることができます。


The following image is an example of a patient being ventilated in Volume-Targeted ventilation. The ventilator settings are: Volume A/C; Respiratory Rate 20/minute; Tidal Volume 0.5 L; Peak Flowrate 60 LPM; Decelerating Flow Waveform; PEEP 0 cm H2O. This patient is not making any spontaneous efforts.

 以下の図は容量規定換気によって換気されている患者の例です。人工呼吸器の設定は、容量規定のアシストコントロール、呼吸回数20回/分、一回換気量0.5L、最高流速60LPM、漸減波、PEEP 0 cm H2Oです。この患者は自発呼吸の努力をしていません。



The Flow versus Time Waveform is the second tracing. Notice how the tracing on the graph goes up to the set peak flowrate of 60 LPM, then gradually decreases throughout inspiration because the decelerating ramp flow waveform was selected. When expiration begins, the tracing demonstrates the peak expiratory flow of approximately 50 LPM, and then the tracing gradually returns back up to baseline at the end of expiration. Because the expiratory flow returns to baseline, the patient has no AutoPEEP.

 上から2つめがフロー−時間波形です。漸減波形が選択されているためにグラフ上のトレーシングが設定のフローである60LPMに達し、その後、徐々に吸気の間中減少していることに注目してください。呼気が開始した時、最高呼気速度がおおむね50LPMであり、その後トレーシングは徐々に呼気の終末に基線に戻ってきているのを表しています。呼気フローが基線に戻っているため、この患者にはオートPEEPはありません。

Pressure Spike
圧波形における“スパイク”現象

When you look at the Pressure versus Time Waveform in the following image, you notice that the tracing for the first two breaths appears normal. However, on breaths three and four, there is a sudden increase in pressure toward the end of inspiration. This is referred to as a Pressure Spike. It represents a sudden increase in pressure at the end of inspiration.

 次の圧-時間波形を見ると、はじめの2波形は正常ですが3つ目及び4つ目の波形では吸気後半にかけて圧の上昇が突然見られることに気づくでしょう。これが圧波形の“スパイク”とよばれるものです。吸気終了時点での突然の圧の上昇が起こっています。


There are two primary causes of a Pressure Spike. One cause of a Pressure Spike is that the patient is activating his or her expiratory muscles while the ventilator is still delivering gas in the inspiratory phase. Generally, this indicates that the inspiratory time is too long and should be reduced. This can also be detected by placing your hand on the patient’s abdomen, which will allow you to feel contraction of the expiration muscles while the ventilator is still delivering the breath.

 この現象は2つの要因から起こります。まずひとつは人工呼吸器がいまだ吸気相でガスを送り続けようとしている際に、患者自身が呼気筋を使って息を吐こうとしている状態です。一般的に、この状態は吸気時間が長すぎること示しており、吸気時間を短縮する必要があります。患者の腹部に実際手を置いてみると、人工呼吸器が送気をし続けている間に患者自身が呼気筋を使用し息を吐こうとしていることを感知することができます。

The second cause of a Pressure Spike is that the delivered tidal volume is too high. If the delivered tidal volume is too high, the compliance of the lung and thorax will suddenly decrease at the end of inspiration. This results in a sudden increase in pressure which is displayed on the graphic waveform as a Pressure Spike.

 圧-時間波形における吸気後半でのスパイク波形のもうひとつの要因に、供給一回換気量が大きすぎることがあげられます。供給一回換気量が大きすぎると、肺と胸郭のコンプライアンスが吸気終末で突然低下します。これに伴い圧が突然上昇し、グラフィック波形に圧スパイクとして表れるのです。

Sagging Pressure versus Time Waveform

When you look at the following image, you notice that the Pressure versus Time Waveform on the second and third breaths does not demonstrate the expected gradual increase in pressure as discussed earlier. In fact, the waveform actually demonstrates a decrease in pressure or a Sagging appearance in the middle of the breath. This is usually referred to as a Sagging Pressure versus Time Waveform and it means that the patient’s inspiratory effort has exceeded the set flowrate and/or the set tidal volume, resulting in patient-ventilator dys-synchrony.

 次にあげる画像を見た際、2つめと3つめの呼吸では前述したような段階的な圧の上昇がみられないことに気付かれるでしょうか。実際この波形は呼吸の途中で圧が低下し、陥没した波形になっています。これは陥没波形といわれ、患者の吸気努力が設定した吸気流速や一回換気量を上回り、結果的に人工呼吸器との非同調をおこしていることを表します。

Part 2 – Pressure versus Time Waveform
パート2 圧-時間波形

The Pressure versus Time Waveform is used to help identify patient-ventilator dys-synchrony. Some published research has identified patient-ventilator dys-synchrony in up to 25% of patients receiving ventilatory support. Patient-ventilator dys-synchrony may increase the patient’s work-of-breathing, prolong ventilatory support and may increase the chance of ventilator-induced lung injury. Therefore, it is very important to carefully monitor the Pressure versus Time Waveform to prevent patient-ventilator dys-synchrony.

 圧―時間波形は患者と人工呼吸器の非同調を把握するのに役立ちます。数点の研究で人工呼吸器管理を受けている患者の最大25%に人工呼吸器との非同調があると発表されました。患者と人工呼吸器の非同調は呼吸仕事量を増やし、人工呼吸器装着期間を延長し、VILI(人工呼吸による肺障害)を起こします。したがって、圧-時間波形を注意深く観察し、患者と人工呼吸器の非同調をふせぐことがとても重要なのです。

The following image is an example of a patient being ventilated in Volume-Targeted ventilation. The ventilator settings are: Volume A/C; Respiratory Rate 20/minute; Tidal Volume 0.5 L; Peak Flowrate 60 LPM; Square Flow Waveform; PEEP 0 cm H2O. This patient is not making any spontaneous efforts.

 次の画像は容量規定換気で人工呼吸器管理下にある患者の例です。人工呼吸器設定はVolume AC換気回数20/分、一回換気量500ml、最大吸気流速60LPM矩形波、PEEP 0cmH2Oで自発呼吸はありません。



The Flow versus Time Waveform is the second tracing. Notice how the tracing on the graph goes up to the set peak flowrate of 60 LPM, then gradually decreases throughout inspiration because the decelerating ramp flow waveform was selected. When expiration begins, the tracing demonstrates the peak expiratory flowrate of approximately 50 LPM, then the tracing gradually returns back up to baseline at the end of expiration.

 2段目の波形が流量-時間波形です。グラフィック上で設定された60LPMのところまで曲線が上がり、そこから吸気終了までゆっくりと低下することが見てわかるでしょう。これは漸減波に設定されているためです。呼気が開始すると、曲線は呼気流速のおおよそ50LPMで頂点に達し、呼気の終了までゆっくりと基線に戻ります。

The Volume versus Time Waveform is the bottom tracing. It gradually increases to the set tidal volume of 0.5 L, then returns to baseline at the end of expiration.

 最下段が容量-時間波形です。設定した換気量500mlに到達するまでゆっくり曲線が上昇し、呼気終了の時点で基線に戻ります。

The Pressure versus Time Waveform is the top tracing. At the beginning of inspiration, the pressure increases rapidly to about 8 cm H2O due to the resistance of the circuit and endotracheal tube. Then, because the Flow versus Time Waveform is decelerating, the flowrate decreases, and the rise in the inspiratory pressure up to 15 cm H2O is not as quick, resulting in a reduction of the slope of the Pressure versus Time Waveform. During exhalation, the tracing returns to baseline, which is 0 cm H2O because no PEEP was selected. The shape of this Pressure versus Time waveform is typically referred to as a tapered or shark-fin waveform, and is a normal Pressure versus Time Waveform when ventilating a patient in Volume-Targeted Ventilation with a decelerating flow waveform.

 最上段が圧-時間波形です。吸気開始時点で回路と挿管チューブの抵抗により圧がおおよそ8cmH2Oのところまで急速に上昇します。流速-時間波形は漸減波を選択しているため流速は低下を呈し、圧波形で吸気圧が15cmH2Oへ上昇する傾きはゆるやかです。呼気曲線は基線へ戻り、この場合はPEEPが設定されていないので0cmH2Oとなります。この圧-時間波形の曲線はサメのひれに形容され、漸減波を用いた際の量規定の正常な圧―時間波形です。
